The following are sample reports available to government health departments using the Dynamic SF-36.

3 Year Health Plan
Dashboard Indicator
This report allows you to represent 3 yearly health outcomes targets for patients in your survey group. Identify targets for each associated health risk factor.

Annual Health Outcome Targets
Dashboard Indicator
Compare actual health outcomes to annual targets. These reports can be presented as a total summary, or summary by age group, health risk factor etc.

Process Oriented Health Outcomes Targets
Dashboard Indicator
Compare actual versus targeted health outcomes for specific health intervention or treatment process (e.g. Hospitalisation).

Cascading Indicator
These reports display quarterly versus annual health outcomes for your survey group. You can drill-down through the data to examine national, state area health services, hosptials, divisions, practitioners and individual patients.

Patient SF-36 Scores Summary with Graphs
This report displays the Physical and Mental Health Summaries for all surveys for a patient. To view the acutal scores of a survey and its preceding 2 surveys, click on a survey date in the list.

Complete this background survey to generate a summary report containing patients name, age, nutritional status, body mass index and the most recent SF-36 scores.

GP Divisions Report Card
Shows the average test scores for GP Divisions represented in your survey group. You can use this report to get a ‘snapshot’ picture of the groups fitness level and health status.
